Hospice Service Staff

About our Staff

Family Comfort Hospice is available 24 hours a day, 7 days a week to provide a comprehensive range of hospice services to fit the needs of each individual. We adapt as a patient’s needs change and work closely with family members to determine what is required. Our approach is interdisciplinary and our collaborative team includes a wide range of specialty and expertise, including:

Attending Physician

The attending physician is a key member of the Hospice Team, assuming responsibility for medical management of the patient’s treatment throughout their involvement in the hospice program.

Hospice Medical Director

Our medical director works closely with your personal physician and staff, providing the expertise and perspective of hospice care.

Clinical Director

Our Clinical Director is a registered nurse who is responsible for managing and coordinating the care between the patient and the interdisciplinary team (IDT). He/she makes certain that the plan of home care for each patient is unique, individualized and interdisciplinary. He/she is often the most accessible by phone and coordinates the physician’s orders with the other team members who will carry them out.

Registered Nurse (RN)

Our hospice nurse visits the patient wherever they reside. The number of visits depends on the needs of the patient, providing direct care to the patient and keeping the doctor informed and updated about the patient’s condition. Our nurse also teaches the family and/or other caregivers how to care for the patient.

Social Worker (MSW)

Our social worker provides direct support and counseling for the patient, family members, and caregivers. The social worker (MSW) also serves as the primary contact to help the patient and family access community resources.

Home Health Aides/Certified Nursing Assistants (CNA)

Our aides and assistants provide personal care such as bathing and grooming, under the direction of the RN, as well as assistance with other activities of daily living. The frequency of aide visits are based on the needs of the patient and the caregiver, and will be adjusted accordingly throughout the entire admission.

Hospice Chaplain / Bereavement Coordinator

The chaplain is available to help the patient and family members with spiritual and religious matters. He/she is non-denominational, and can serve as liaison to the family’s clergyman.

Hospice Care Volunteers

Our volunteers may provide emotional support and companionship to patients and their families. They can also provide respite for caregivers and family members involved in the patient’s care.
