What’s Palliative Care?
Palliative care is an umbrella that allows us to follow our patients throughout their journey of care. When an individual enters on our services, the Palliative program will monitor their progress, help match them with potential movement for additional care through our partners, and will provide follow up by phone and newsletters to keep them connected to our staff. This program allows us to build a strong bond where we can care and support your family and your loved ones through their health experience, and when they enter another stage of their journey, we are able to assist you as necessary and be here to answer all questions you may have.
By using the Palliative program, we can provide the you and your family with seamless and simple medical equipment solutions, continuous access to our pharmaceutical partners to avoid lapses and challenges in prescriptions and we will be able to manage comprehensive electronic medical records to keep all patient information easily accessible in a simple one-source access point if you need additional care and support services. Visiting doctors will ease potential communication breakdowns and allow you and your family to enjoy a strong relationship with our network. We will also be a resource from a non-medical standpoint as well; we can provide recommendations for handymen, community resources, family and personal assistance, and much more. The Palliative program will allow us to be deeply connected with you and to build strong, productive relationships that allow us to give more fully to you and others that we serve in the community.
