Position: Registered Nurse Case Manager

Job Summary:
The RN Case Manager continuously plans, implements, and evaluates patient care, incorporating patient and family into the process, provides direct patient care, and functions as a patient advocate. Responsible for participating in the development of an interdisciplinary plan of care and ensuring the overall coordination and management of hospice services for patients and their families.

Job Description:
• Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for hospice nursing.
• Collaborates with IDG, other healthcare professionals, patients, caregivers in developing in ongoing IDG comprehensive assessments.
• Based on the comprehensive assessment by and in coordination with the Interdisciplinary Group (IDG), develops and evaluates Plan of Care (POC) for assigned patients with patient, family and attending physician
• Documents all findings, plans, interventions and interactions with patients and families in the clinical record according to policy, procedure and guidelines.
• Demonstrates competency with nursing skills within professional standards of practice
• Performs comprehensive assessments of patient status, including physical, psychosocial, spiritual and environmental factors.
• Assessment skills as applied to patient, caregiver and environment
• Teaching skills according to the patient’s needs and within the context of the patient’s environment and physical/emotional status
• Knowledge of medications and medication administration
• Provides ongoing education to patients and families on plan of care interventions
• Observe, assess and documents symptoms of hospice patients accurately, timely and according to policy, procedures and guidelines
• Provides emotional support to patient and caregiver(s)
• Meets safety needs of patient and demonstrates safe practice in the use of equipment
• Contributes to patient and family counseling and education.
• Participates in on-call schedule.
• Participates in hospice sponsored in-service training.
• Participates in QAPI program.
• Performs job duties in accordance with Hospice policies, procedures, and professional and community standards, state and federal regulations.
• Abides by the Hospice’s Standards of Conduct.
• Performs other related duties and responsibilities as assigned.

Minimum Qualifications:
• A current license as a registered nurse.
• Must have a valid driver’s license and proof of automobile insurance

Preferred Qualifications:
• Minimum of one (1) year experience in hospice, acute care setting, nursing home or home care
• Hospice experience preferred
• Demonstrated written, verbal, and interpersonal communication skills

Position: Hospice Aid

Job Summary:
Provides personal care services to patients to maintain personal comfort under the direction of the Registered Nurse.

Job Description
• Performs personal care activities including bathing, dressing, grooming, eating, ambulating and toileting as assigned by the RN
• Performs simple procedures under the direction of the RN such as, range of motions, assistance in ambulation or exercises.
• Performs homemaker services, including, but not limited to: meal preparation, laundry, or light housekeeping in the immediate patients’ living area.
• Documents all findings, plans, interventions and interactions with patients and families in the clinical record.
• Meets safety needs of patient and demonstrates safe practice in the use of equipment
• Performs other related duties and responsibilities as assigned.

Qualifications
Mandatory:
• Ability to read and write English.
• Be at least 18 years of age
• Must meet Federal and state Hospice Aide training and competency requirements.
• One year of recent experience working as a Certified Home Health Aide without a lapse of 24 consecutive months without services provided for compensation.

Desired:
 Certified Nursing Assistant with current Arizona certification.
• Knowledge of home care principles and practices within the scope of hospice.
• Demonstrated written, verbal, and interpersonal communication skills
• Must possess a valid Arizona driver’s license and proof of automobile insurance.
